WTF: Wellness Tracking Framework (“WTF”) is an early beta prototype designed to help users organize lab results, symptoms, medications, vitals and measures, health events, and appointment questions into a personal health timeline.

WTF is intended for personal tracking, organization, education, and appointment preparation only.

Not medical advice

WTF does not provide medical advice. The information, summaries, prompts, lab descriptions, trend views, and appointment-preparation tools in WTF are provided for general informational and organizational purposes only.

WTF does not diagnose, treat, prevent, monitor, or cure any disease or health condition. WTF does not recommend medication changes, treatment decisions, testing decisions, or clinical action.

Always seek advice from a qualified health professional about your health, symptoms, medications, lab results, or medical concerns.

Not a substitute for clinical care

WTF is not a substitute for care from a physician, nurse practitioner, pharmacist, nurse, or other qualified health professional.

Do not ignore, delay, or avoid medical care because of information entered into, displayed by, or generated through WTF.

Not for emergencies

WTF is not designed for urgent or emergency situations.

If you are experiencing severe symptoms, chest pain, severe shortness of breath, fainting, confusion, signs of stroke, severe allergic reaction, suicidal thoughts, or any other urgent or concerning symptoms, seek emergency medical care or contact local emergency services immediately.

No clinician-patient relationship

Use of WTF does not create a clinician-patient, advisor-client, or professional relationship with the creator of WTF.

Submitting feedback, sending email, or using the beta prototype does not create a clinical care relationship.

Lab results and health information require context

Lab results, symptoms, medications, and home readings are interpreted in context. Reference ranges may vary by laboratory, method, age, sex, pregnancy status, medication use, medical history, and other clinical factors.

WTF may help you organize information and notice patterns, but it cannot determine what those patterns mean for your individual health.

Accuracy and reliability

WTF is currently in early beta. Features may change, and occasional errors, bugs, browser differences, or display issues may occur.

While care has been taken to use calm, general, health-safe language, WTF may contain incomplete, inaccurate, outdated, or unclear information. You are responsible for reviewing your entries and confirming health information with a qualified health professional.

Data entry and user responsibility

You are responsible for the accuracy of the information you enter into WTF.

WTF does not verify your lab values, medication doses, symptoms, reference ranges, units, or clinical notes. Before using any information from WTF in a health-care conversation, review it for accuracy.

No warranties

WTF is provided “as is” and “as available” as an early beta prototype. No warranties are made about its accuracy, completeness, reliability, fitness for a particular purpose, uninterrupted availability, or suitability for any medical or clinical use.

Limitation of liability

To the fullest extent permitted by applicable law, the creator of WTF is not responsible for any loss, harm, injury, damages, data loss, decisions, delays in care, or other consequences arising from use of, reliance on, or inability to use WTF.

Use at your own discretion

By using WTF, you understand that it is a personal tracking and appointment-preparation tool only. You agree not to rely on WTF as medical advice, diagnosis, treatment guidance, or your only record of health information.
